August 30, 2014 ANTHONY SCOTT POLAKOV [#], 58, of Los Angeles, was placed on one year of probation and faces a one-year suspension if he does not comply with the terms of his probation. He was also ordered to take the MPRE. The order took effect Aug. 30, 2014. Polakov stipulated to committing moral turpitude in one client matter. Polakov represented a client in a workers’ compensation matter arising from a 2009 injury. In March 2011, he took various documents to the Workers’ Compensation Appeals Board office for filing but realized that the original signed copies were still at his office. He then proceeded to sign the documents for himself and for the client. Among them was a declaration attached to the workers’ compensation claim form that he signed as his client under penalty of perjury. In mitigation, Polakov did not harm his client, entered into a pretrial stipulation with the State Bar, presented evidence of his good character and had no prior record of discipline. |
